Defektbezogene Konzepte in der adhäsiven postendodontischen Restauration

Zusammenfassung

Beim Wiederaufbau endodontisch behandelter Zähne haben die Faktoren Verlust gesunder Zahnhartsubstanz, Zahntyp, Ausmaß der Ferrule-Präparation, das Bestehen von Approximalkontakten sowie die Restaurationsart einen entscheidenden Einfluss auf die Überlebensrate dieser Zähne. Lediglich bei einem hohen Zerstörungsgrad der Zähne mit noch einer bestehenden Kavitätenwand mit einer Dentinbasis von mindestens 1 mm oder äquigingivalen Defekten ist die adhäsive Insertion eines Wurzelstifts indiziert. Darüber hinaus ist es von Bedeutung, dass die verbliebene gesunde Zahnhartsubstanz durch adhäsive und/oder mechanische Maßnahmen, wie die Präparation eines 2 mm starken „ferrule design“ bei einer Kronenversorgung stabilisiert sowie maximal geschont wird. Bei der Auswahl des Wurzelstifts gelten faserverstärkte Stifte als bester Kompromiss in Bezug auf die biomechanischen Eigenschaften, Ästhetik und Befestigungsoptionen einerseits und mechanischer Stabilität andererseits.
