Schlafmedizin als interdisziplinäre Aufgabe aus Sicht des Neurologen

Zusammenfassung

Die Mehrzahl der in der internationalen Klassifikation für Schlafstörungen (ICSD)-2 aufgeführten Erkrankungen bzw. Schlafstörungen fallen in das Fachgebiet der Neurologie. Die schlafbezogenen Atmungsstörungen (SBAS) stellen die häufigsten Schlafstörungen dar und haben ebenfalls in vielen Fällen neurologische Erkrankungen als Ursache oder zumindest bedeutsame neurologische Teilaspekte. Angesichts der Tatsache, dass unterschiedliche Schlafstörungen (Insomnie, Parasomnie, SBAS, Restless-Legs-Syndrom) jeweils für sich genommen eine hohe Prävalenz in der Bevölkerung aufweisen, ist davon auszugehen, dass sie bei vielen Patienten gemeinsam vorliegen und sich gegenseitig beeinflussen können. Die Interdisziplinarität der Schlafmedizin verlangt zunehmend, die Betrachtungsweise von Schlafstörungen aus der Perspektive nur eines Einzelfachs zu verlassen. Die Schlafmedizin ist ein Querschnittsfach, das die jeweils relevanten Bereiche aus der Neurologie, Pneumologie, Psychiatrie, Hals-Nasen-Ohren-Heilkunde und MKG-Chirurgie zusammenführt. Umfassend ausgebildete Schlafmediziner in ausgewiesenen Zentren sollten in der Lage sein, alle nach ICSD-2 klassifizierten Schlafstörungen zu diagnostizieren und zu therapieren bzw. eine adäquate Therapie in die Wege zu leiten. Nur durch interdisziplinäre Kooperation kann gewährleistet werden, dass im Sinne der Schlafgesundheit die verschiedenen Aspekte schlafmedizinischer Erkrankungen angemessene Berücksichtigung finden.

Prof. Dr. med. Gregor Antoniadis Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
