Schnittführung in der Implantologie

Zusammenfassung

Bei der Schnittführung wird mit Rücksicht auf den histologischen Aufbau der Mundschleimhaut die Schnitttiefe so festgelegt, dass die vaskuläre Versorgung in der Lamina propria nicht gefährdet wird. Die Lokalisation und horizontale Ausdehnung der Schnitte sollte sich an den vaskulären Versorgungszonen (Angiosomen) der Mundschleimhaut orientieren, um eine primäre Wundheilung zu ermöglichen und das Ausschneiden avaskulärer Segmente zu vermeiden. Verschiebungen und Veränderungen der roten Farbschichtung von Mukosa und Gingiva sollten vermieden werden, um die Ästhetik durch unvorteilhafte Narbenbildung nicht negativ zu beeinflussen. Daher sind midkrestale Schnitte (Kieferkammschnitte) auf dem zahnlosen Alveolarfortsatz zu bevorzugen, marginale Schnitte in den angrenzenden bezahnten Bereichen. Sie ermöglichen in den meisten Fällen eine ausreichende Sicht auf das Operationsfeld. Kombiniert mit einer Periostschlitzung wird genug Platz für Augmentate und eine ausreichende Mobilisation zur Deckung des Operationsgebiets erzielt. Entlastungsschnitte ins Vestibulum sind möglichst zu vermeiden. Aufgrund fehlender operativer Techniken zur gezielten Beeinflussung der roten Weichgewebsfarben ist dem Erhalt einer unveränderten Weichgewebsarchitektur höchste Priorität einzuräumen.
