Zur Vorhersagbarkeit von Suszeptibilitätsartefakten durch metallische orthodontische Apparaturen in der Magnetresonanztomographie

Zielsetzung

Kieferorthopädische Apparaturen werden vor MRT-Untersuchungen oft präventiv entfernt, mitunter aber auch aus Unwissenheit intraoral belassen. Beides kann ungünstige Folgen für die Patienten haben: Entweder entstehen vermeidbare Kosten und ein erheblicher Behandlungsaufwand beim Zahnarzt oder es kommt zu einer Artefaktentstehung, welche die diagnostische Aussagekraft der MRT signifikant beeinträchtigt. Ziel dieser Arbeit ist es, die erzeugten Artefaktgrößen durch orthodontische Apparaturen experimentell zu bestimmen und anhand einer fundierten werkstoffkundlichen Recherche Kriterien zur MRT-Kompatibilität zu erarbeiten, die Radiologen und Kieferorthopäden behilflich sein könnten.

Methoden

Die Exposition der insgesamt 16 Objekte aus Stahl-, Titan- und CoCr-Legierungen erfolgten in einem mit Wasser gefüllten, gekammerten Phantom bei 1,5 und 3 T. Es wurden jeweils Spinecho- und Gradientenechosequenzen durchgeführt.

Ergebnisse

Die Artefakte hängen von Materialeigenschaften (Probengröße, Gefüge und Verarbeitungsweise durch den Hersteller) sowie den Kennwerten des Tomographen (Stärke des Hauptfelds, Sequenz) ab. Bei den verschiedenen Stahlsorten variieren die Ergebnisse deutlich. Es zeigten sich Artefaktradien zwischen 14 mm (1,5 T/Spinecho) und 51 mm (3 T/Gradientenecho). Um die Titan- und CoCr-Proben traten beim Hauptfeld von 1,5 T keine Artefakte auf, ebenso bei einer Stahlsorte.

Schlussfolgerung

Anhand der Bezeichnung Stahl lässt sich die Artefaktgröße nicht vorhersagen. Auch das Gefüge des Ausgangsmaterials zur Produktion der Stahlapplikationen spielt eine untergeordnete Rolle. Der relevante physikalische Parameter ist die magnetische Permeabilität (bzw. Suszeptibilität) des Endproduktes, der jedoch durch die Hersteller nicht deklariert wird und am intraoralen Objekt nicht messbar ist. Die vorgestellte Studie zeigt, dass es auch Produkte aus Stahl gibt, die unbedenklich im Mund belassen werden können.
