A 46-year-old woman presented to an external hospital because of upper abdominal pain and nausea. A suspected case of pyelonephritis was diagnosed. The increasing loss of alertness was attributed to a combination of sepsis and hepatic encephalopathy in the context of pre-existing cirrhosis due to known autoimmune hepatitis. Protective intubation was performed, and the patient was transferred to our center for treatment of grade 2 acute-on-chronic liver failure (ACLF). …
