Recently, six large ischemic core trials, RESCUE Japan LIMIT, SELECT 2, ANGEL-ASPECT, TESLA, TENSION and LASTE, which broadly included patients with an ASPECTS of 0–5 on computed tomography (CT) or magnetic resonance imaging (MRI), demonstrated significantly improved functional outcomes and comparable safety profiles amongst patients who underwent endovascular thrombectomy (EVT) compared to those without EVT treatment [

6]; however, the treatment efficacy for patients with an extensive infarct size (ASPECTS 0–2) remains undetermined as only the LASTE trial permitted recruitment of patients between ASPECTS 0–2 (56% of the total cohort), whilst only a small proportion of patients in the remaining individual trials and pooled individual data from the HERMES collaboration were deemed to have a baseline ASPECTS of 0–2 following core laboratory adjudication [
