Zusammenfassung

Die Radiologie von Osteomyelitis und Spondylitis wird – wie die Klinik – ganz von der Akuität der Erkrankung und von der Art des Keims geprägt. Doch gibt es auch abakterielle entzündliche Knochenerkrankungen, wie z. B. die chronische nichtbakterielle Osteomyelitis (CNO) oder die chronisch rekurrierende multifokale Osteomyelitis (CRMO) bei Kindern und Jugendlichen sowie die Ostitis deformans Paget, die im europäischen Raum keine seltene Knochenerkrankung darstellt. Während bei den Osteomyelitiden und Spondylitiden die Diagnosen häufig erst mit Hilfe von Szintigrafie, MRT, CT und CT-gesteuerter Biopsie gestellt werden, spielen moderne radiodiagnostische Methoden beim M. Paget keine wesentliche Rolle.

In den Epiphysen beginnende Osteomyelitiden bei Kindern sind selten, kommen aber vor (Differenzialdiagnose: Osteoidosteom, Langerhans-Zell-Histiozytose-Herd bzw. eosinophiles Granulom).

Hierbei handelt es sich um einen röntgenmorphologischen Begriff mit vielgestaltiger Ätiologie (z. B. subakute bis chronische Spondylitis/Spondylodiszitis; überschießende reaktive Sklerose bei reiner Osteochondrose; unspezifische Spondylodiszitis bei Erkrankungen aus dem rheumatischen Formenkreis; Osteoidosteom).
