Zusammenfassung

Bedingt durch die Umstrukturierungen der kassenärztlichen Notfallversorgung und den demographischen Wandel steigt die Zahl der notfallmäßigen Vorstellungen in Krankenhäusern kontinuierlich. Ein bedeutender Anteil dieser Notfälle betrifft die Fachdisziplin Urologie. Das klinische Erscheinungsbild in der notfallmedizinischen Versorgung reicht dabei von traumatisch bedingten Verletzungen des Urogenitaltrakts und Harntransportstörungen über Hämaturien bis hin zu vital bedrohlichen Infektionen. Durch die hohe Inzidenz urologischer Tumoren und die Affektion des Urogenitaltrakts bei anderen malignen Erkrankungen sind zudem onkologische Patienten häufig von urologischen Notfällen betroffen. Ein schnelles und effizientes diagnostisches sowie therapeutisches Handeln ist in diesen Situationen entscheidend, um ein optimales Behandlungsergebnis zu erzielen.

Um die Langzeitfolgen eines Delirs bei kritisch Kranken zu mildern, wird vielerorts auf eine Akuttherapie mit Antipsychotika gesetzt. Eine US-amerikanische Forschungsgruppe äußert jetzt erhebliche Vorbehalte gegen dieses Vorgehen. Denn es gibt neue Daten zum Langzeiteffekt von Haloperidol bzw. Ziprasidon versus Placebo.
