Typische Verletzungen durch terrorassoziierte Ereignisse und ihre Implikationen für die Erstversorgung

Zusammenfassung

Terrorassoziierte Ereignisse oder Amoklagen gehen, wenn Kriegswaffen und Sprengmittel verwendet werden, mit Schuss- und Explosionsverletzungen einher. Da diese Ereignisse und die typischen Verletzungen in Deutschland trotz einer wahrnehmbaren terroristischen Bedrohungslage selten sind, können die Besonderheiten der Behandlung nicht vollumfänglich vorausgesetzt werden. In der Taktik und Strategie der klinischen Versorgung der Verletzten in einer Schadenslage nach Anschlag oder Amok ist bis zum Vorliegen des kompletten Lagebildes von einer vorübergehenden Mangelsituation auszugehen. In der initialen Phase ist damit zu rechnen, dass zunächst Patienten mit nicht beherrschbaren Blutungen an Körperstamm und Körperhöhlen das erstversorgende Krankenhaus erreichen. Diesen ist nur ein Überleben zu ermöglichen, wenn die chirurgische Versorgung auf das Nötigste fokussiert wird („tactical abbreviated surgical care“). Nach der Stabilisierung dieser Verletzten und Vorliegen aller Lageinformationen kann dann die Versorgung und Rekonstruktion der Schuss- und Explosionsverletzungen nach den klinischen Kriterien des Damage Control Surgery (DCS) erfolgen. Der vorliegende Beitrag gibt hierzu eine Übersicht über die Pathophysiologie der Wundentstehung, Wundballistik und die erfolgreich angewendeten Prinzipien und Vorgehensweisen der Behandlung.
