25. Stentimplantation: selbstexpandierbare Stentgrafts

Zusammenfassung

Endovaskuläre selbstexpandierbare Stentgrafts können neben der klassischen Indikation der Aneurysma-Ausschaltung und Überdeckung von Gefäßlazerationen mit Blutung auch zur Behandlung einer arteriellen Verschlusskrankheit im iliakalen und femoropoplitealen Stromgebiet eingesetzt werden. Nach Einführung moderner Stents und der medikamentenfreisetzenden Ballons ist der Stentgraft Methode der 2. Wahl in der primären Therapie und bei der In-Stent-Restenose. Der Stentgraft kann bei akuten Verschlüssen, insbesondere bei koinzidentem (v. a. Popliteal-) Aneurysma, primär anstelle oder in Kombination mit einer Lysetherapie eingesetzt werden. Die Edge-Stenose ist die wichtigste die Offenheitsrate beeinflussende Komplikation und bedarf besonderer Beachtung. Bei der Implantation gilt es, auf eine exakte Dimensionierung und die Einhaltung von Hygieneregeln zu achten.
