A 37-year old man, with surgically corrected congenital heart disease (repaired tetralogy of Fallot [rTOF] and subsequent biological pulmonary valve replacement with patch augmentation to pulmonary trunk and right ventricular outflow tract [RVOT]), was referred to our institution to undergo ventricular tachycardia (VT) ablation for recurrent syncopal VT episodes. The majority of VTs in rTOF are related to right ventricular (RV) anatomically defined isthmuses [
