Postural orthostatic tachycardia syndrome (POTS) is a chronic form of orthostatic intolerance characterized by excessive orthostatic tachycardia in the absence of orthostatic hypotension [
1,
2]. We read with interest Professor Stewart and colleagues’ manuscript in a recent issue of
Clinical Autonomic Research entitled “Standing tests lack reliability to diagnose all adolescents who have postural tachycardia syndrome” [
3]. In this study, Stewart et al. attempt to assess reliability of an active stand test versus a tilt table test to diagnose pediatric patients with POTS. We applaud the authors’ effort to advance our ability to accurately diagnose patients with POTS, as this is a topic of clinical importance. However, it must be highlighted that the authors’ conclusions rely on circular logic, which limits the conclusions and study impact. As such, we are not persuaded that we should dismiss the active stand protocol so quickly. …
