Zusammenfassung

Die Polymyalgia rheumatica (PMR) ist die häufigste entzündliche Autoimmunerkrankung im höheren Lebensalter mit der höchsten Inzidenz im Alter von etwa 73 Jahren. Typische Symptome sind akut/subakut auftretende beidseitige Schmerzen im Schultergürtel-, Nacken- und/oder Beckengürtelbereich, die mit einer ausgeprägten Morgensteifigkeit einhergehen. Eine Riesenzellarteriitis (RZA) tritt bei etwa 20 % der Patienten auf; bis zu zwei Drittel der RZA-Patienten haben eine PMR-Symptomatik. Verschiedene Erkrankungen können eine PMR imitieren, insbesondere die rheumatoide Arthritis des höheren Lebensalters. Es gibt keinen für die PMR spezifischen Labortest, in >90 % der Fälle sind die Blutsenkungsgeschwindigkeit und/oder das C‑reaktive Protein erhöht. Zur Diagnostik der PMR können Ultraschall und/oder MRT beitragen. Therapeutisch werden Glukokortikoide angewandt. Die Behandlungsdauer beträgt häufig 2 bis 3 Jahre, manchmal länger. In bestimmten Situationen wird niedrig dosiertes Methotrexat als zusätzliche Therapie verwendet.
